People with ADHD have more than four times the odds of experiencing encopresis, a loss of bowel control, compared to people without the condition. That is the single biggest gap uncovered in a new analysis combining data from 23 studies and more than 1.9 million people.

Published in the Journal of Attention Disorders, the review also found that people with ADHD had roughly 48% higher odds of gastrointestinal symptoms overall compared to people without ADHD. Its authors describe it as the first review of its kind to measure how common these gut symptoms are in people with ADHD, rather than just counting formal digestive disease diagnoses.

Gut symptoms turned up across the board, from irritable bowel syndrome to ongoing stomach pain. For a condition long associated mainly with focus, impulsivity, and behavior, that pattern points to a physical side that has mostly gone unnoticed.

How Researchers Uncovered the ADHD-Gut Connection

To reach these conclusions, a team of researchers searched through medical research databases looking for every study that examined both ADHD and digestive symptoms. They started with 9,613 potential studies and narrowed that number down through a careful screening process, eventually landing on 23 studies that fit their criteria. Two independent reviewers screened each study, and they resolved disagreements through discussion.

Included studies spanned people of all ages, starting as young as 2 years old, and came from 11 countries including the United States, Sweden, Spain, Germany, China, and South Korea. Sample sizes varied wildly, with some studies including just 50 participants and one including nearly 743,000. To count in the ADHD group, participants needed a formal diagnosis based on recognized medical criteria, not just a self-reported hunch that they might have the condition. The researchers excluded people with diagnosed digestive diseases, since they wanted to isolate everyday gut symptoms rather than separate digestive illnesses.

After gathering the studies, researchers combined the data using a statistical method that calculates an average effect while accounting for differences between studies, such as how ADHD was diagnosed or how gut symptoms were measured. This produced a combined result for each symptom category.

What the Data Show on ADHD and Gut Symptoms

Across the combined data, ADHD was linked to higher odds of every single gut symptom the researchers looked at. The odds of total gastrointestinal symptoms were 48% higher. Irritable bowel syndrome odds were 58% higher. Constipation odds were 97% higher, nearly double those seen in people without ADHD. Odds of dyspepsia, a term for ongoing stomach discomfort or indigestion, were 52% higher. Diarrhea odds were more than double. Odds of abdominal pain were 88% higher. Odds of unspecified stomach and bowel problems were 82% higher. And encopresis, the standout finding, carried more than four times the odds compared to people without ADHD.

One study could not be folded into the main analysis because it used a different scoring system, but it still found a clear pattern: people with ADHD scored much higher on a gut symptom severity scale than those without the condition.

These symptoms were studied separately from formally diagnosed digestive diseases like inflammatory bowel disease, and they appeared even more strongly linked to ADHD than those diagnosed diseases have been in past research. That distinction matters. People with ADHD do not simply get diagnosed with more intestinal illnesses; they seem to report more everyday digestive discomfort even without a diagnosed digestive disease behind it.

Why ADHD and Gut Problems Might Be Linked

Study authors floated several possible explanations, though they note that their research shows a connection, not a cause-and-effect relationship.

One theory centers on behavior. Emotional ups and downs and stress reactions, both common in ADHD, can increase sensitivity in the gut and are linked to stomach pain in general. Irregular eating patterns, including skipped meals or eating in bursts, are also frequently seen in people with ADHD and have separately been tied to bloating, constipation, and indigestion. Sensory differences may play a role too. Some people with ADHD are more sensitive to food textures and smells, which can lead to picky eating and diets low in fiber, a known cause of constipation.

Another theory points to gut bacteria. Prior research has found that people with ADHD tend to have different gut bacteria compared to people without the condition. The authors note that gut bacteria and the nerve connecting the gut to the brain seem to influence each other, and that changes in this system could lead to more gut inflammation, which in turn could produce more symptoms like diarrhea and constipation.

Medication and sleep were also mentioned as possible contributors. Stimulant medications commonly used to treat ADHD are known to cause appetite loss, nausea, and stomach pain as side effects. Sleep problems, which are common among people with ADHD, can also affect digestion. The researchers pointed out, however, that constipation and encopresis in particular are not symptoms typically caused by stimulant medications. That suggests something beyond medication side effects likely drives at least part of this pattern.

Bottom line, this research does not show that ADHD causes stomach problems in a direct, simple way. What it does show is an overlap strong enough that gut symptoms deserve real attention in ADHD care rather than being brushed off as unrelated. Study authors argue that clinicians should actively ask about digestive symptoms and, in tougher cases, bring in dietitians or gastroenterologists so these issues are not overlooked. Anyone managing ADHD, or caring for a child who has it, may want to raise stomach symptoms with a doctor rather than treating them as a separate concern.

Limitations

Researchers note several limitations. The team excluded studies published in languages other than English because of resource constraints, which may have left out relevant findings from non-English-speaking regions. The team also could not formally assess publication bias, meaning studies with positive findings may have been more likely to be published and included than studies with null results. Additionally, the analysis could not fully explore why results varied so much between individual studies. Most of the included studies did not separate out results based on whether participants were taking ADHD medication, so the researchers could not completely rule out that some gut symptoms were side effects of treatment rather than something intrinsic to ADHD itself. That said, they note that constipation and encopresis, two of the strongest findings, are not symptoms typically linked to stimulant medication, suggesting a component separate from drug side effects. Time constraints also prevented the team from contacting the original study authors to request missing data.
