Fake Reviews Are Winning the Trust War in Healthcare

For many, the search for a new doctor begins with a quick scroll through star ratings and reviews. A new study finds that trust may be misplaced more often than most people realize. Researchers who tracked thousands of online physician reviews, including a rare batch confirmed to be fake, found that the fake reviews received more helpful votes from real users and scored higher on a measure of perceived trustworthiness than the genuine ones.

A research team from Simon Fraser University, publishing their findings in the journal Internet Research, stumbled onto an unusual opportunity. A security flaw on a physician-booking platform in India let clinics and doctors write glowing reviews of themselves while posing as patients. Once the flaw was discovered, the platform could label those reviews as fraudulent with near-certain accuracy, giving scientists something researchers on this topic rarely get: a large batch of confirmed fake reviews sitting next to genuine ones, rather than guesses or stand-ins created by hired writers.

What the team found flips a common assumption on its head. Fake physician reviews were not sloppy or vague attempts to fool readers. They were longer, more specific, and more likely to include personal details about symptoms, medications, and treatment, the very features that make a review feel authentic. Genuine patients wrote reviews that were shorter and less detailed by comparison. That gap, researchers say, is exactly what fraudulent reviews exploit.

Fake Doctor Reviews Looked More Convincing Than Real Ones

Researchers analyzed a random sample of 5,000 physician reviews pulled from a larger set of nearly 35,000, all collected between 2015 and 2017 from the booking platform, which covered doctors across specialties from general physicians and dentists to dermatologists and cardiologists. Roughly a quarter, or about 24 percent, of the sampled reviews were confirmed fake, caught through that same security loophole. Because the platform could trace exactly which reviews came through the loophole, researchers did not have to guess which reviews were real and which were staged, a problem that has limited earlier research on this topic.

Each individual review carried a short block of text and a count of “helpful” votes from other users. To capture trustworthiness, something a vote count cannot measure on its own, researchers had the AI model GPT-4 score every review on a one-to-five scale, then checked its scores against two human raters working independently through a random sample of 500 reviews. The AI and the humans agreed closely enough that the machine’s scores could be trusted.

Researchers then compared fake and genuine reviews across several measures. Fake reviews were longer (about 261 characters versus 206), received more helpful votes, disclosed more personal health information, discussed doctors’ interpersonal skills more strongly, and scored higher on the trustworthiness measure. Every one of these differences held up under statistical testing, meaning they were not the product of chance.

Fake Doctor Reviews Exploit Patient Privacy Concerns

To figure out what was driving the trust gap, the researchers dug into review content itself. Reviews disclosing more personal health information, things like a specific diagnosis, a named medication, or a described symptom, were tied to more helpful votes and higher trustworthiness scores. Reviews that praised a doctor’s interpersonal skills, such as listening carefully or explaining things patiently, boosted trustworthiness scores by a wide margin, more than ten times as much as health details did, but barely affected perceived helpfulness. Reviews that sounded more personal or warm, in other words, scored higher on trustworthiness whether or not that warmth made them any more useful.

Genuine patients, it turns out, are cautious writers. Concerns about privacy keep many real patients from naming their condition, describing symptoms in detail, or mentioning medications by name in a public review. Fake reviews carry no such hesitation. Whoever is writing them, often a clinic staffer posing as a patient, has every incentive to pile on convincing, specific-sounding detail because that detail is what makes a lie read as truth.

That leaves a real patient guarding personal health information looking less believable than an impostor with nothing to hide.

Fake Doctor Reviews Put Patient Safety at Risk

Choosing a doctor based on reviews built to deceive is not a small risk. A misleading review can steer someone toward a provider who is a poor fit or lead them to make a healthcare choice based on false information.

Regulators have started to take notice: the Federal Trade Commission banned the outright buying and selling of fake reviews across industries in 2024, though enforcement grows harder once the reviewers involved live outside U.S. borders. Until platforms find better ways to verify reviews or protect patients who want to share real detail without exposing their identity, the most convincing review on the page may be the one written to fool the reader.
