Women hoping to keep their memory and focus sharp as they age might want to pay attention to timing, not just effort, when it comes to exercise. A new study found that women who started regular aerobic workouts after menopause saw bigger gains in brain skills like planning and multitasking than women who exercised before menopause, or those who stuck to gentle stretching instead.

There’s a serious reason to take a close look at this. Women face roughly double the dementia risk that men do, and researchers have spent years hunting for habits that might close that gap. Exercise has long been a top candidate, but few clinical trials have actually tested whether a woman’s menopause status changes how much her brain benefits from working out. This new study set out to close that gap.

Researchers tracked 93 healthy women, ages 20 to 67, for six months as they did either aerobic workouts or a gentler stretching-and-toning routine. Writing in the Journal of Alzheimer’s Disease, the team found that women who had already gone through menopause and did aerobic exercise showed the biggest gains in a set of mental skills known as executive functions, which include planning and switching between tasks, compared with both the stretching group and with younger, pre-menopausal women who did the same aerobic program.

Why Aerobic Exercise After Menopause Matters for the Brain

About 45% of dementia risk comes from factors people can actually change, and inactivity is one of them. Menopause adds another layer to the problem. Shifting hormone levels during this transition have been linked to hot flashes, sleep trouble, and what many women call brain fog, including trouble focusing, planning, or remembering things. At the same time, women tend to exercise less around and after menopause, often because of uncomfortable symptoms, limited access to gyms, cost, or a packed schedule. That mix, less exercise happening right when the brain may need it most, helped push the research team to look closer.

Inside the Six-Month Aerobic Exercise Study

Participants were recruited through social media and flyers and had to meet certain requirements: healthy, non-smoking, mentally sharp, and starting out with below-average fitness. They were randomly split into two groups, an aerobic exercise group and a stretching-and-toning group, working out four days a week for six months, adding up to 96 total sessions over 24 weeks. Women in the aerobic group used equipment of their choice, such as an elliptical machine, treadmill, or stationary bike, and their workout intensity gradually increased over the weeks, climbing from about 55 to 65% of their max heart rate up to 75%. The stretching group did flexibility work and core-strengthening exercises without weights.

Everyone was tested at the start of the study, then again at three months and six months, with memory and thinking tests, a fitness test, and a blood draw each time. About 82% of the women completed the three-month assessment, and roughly 71% completed the full six-month intervention. No safety problems came up during the workouts. Researchers then compared results between women who hadn’t yet gone through menopause and those who had, accounting for age, education, and each woman’s starting score on the thinking tests.

What the Brain Tests Showed After Menopause

Executive functions, the skills this study focused on, cover things like switching between tasks, resisting distraction, and staying flexible under pressure, all tied to daily independence and to brain regions that tend to fade with age. A statistical model comparing exercise type, time, and menopause status turned up a clear pattern after three months and again after six months: post-menopausal women in the aerobic group improved more than the stretching group, and more than pre-menopausal women doing the same aerobic workouts. That pattern held up in an additional analysis that accounted for age, a sign the menopause effect wasn’t simply age in disguise.

Still, this advantage didn’t show up everywhere. Processing speed, the other main skill researchers tracked, showed no such pattern. Neither did memory, attention, working memory, or language skills, which were checked as a follow-up. The benefit was narrow, but it lined up specifically with executive functions, the same skills earlier work from this research team flagged as most responsive to aerobic exercise in general.

A Hormone Twist

One idea worth testing was whether aerobic exercise helps the post-menopausal brain by raising estrogen levels. The blood work didn’t support that. Estradiol, a form of estrogen, didn’t change much after either exercise program, and researchers found no link between hormone shifts and the thinking improvements they saw. Whatever drove the benefit, it likely wasn’t a rise in estrogen. Researchers pointed to other possibilities, such as better blood flow to the brain or improved cardiovascular fitness, though this analysis didn’t pin down the mechanism. Post-menopausal women in the aerobic group also started out with the weakest executive function scores of any group in the study, which researchers noted may have simply left more room for improvement.

Sample size was small, and this analysis was a second look at data originally collected for a broader exercise and brain-health trial rather than a study built from scratch to test menopause effects. Even so, the pattern held up across several statistical checks, which gives it some weight.

This trial doesn’t prove that menopause flips some switch that makes aerobic exercise more powerful for the brain. What it does show is that a woman’s hormonal stage in life is a factor exercise researchers have mostly overlooked. Women already face a higher lifetime dementia risk than men, and menopause reshapes brain function for millions of them every year. Figuring out when exercise pays off the most could matter just as much as simply telling women to get moving.

Limitations

This analysis was a secondary, after-the-fact look at data from a trial that was not originally designed to test how menopause interacts with exercise effects on the brain, which limits its statistical power even though the sample size falls within accepted guidelines for pilot trials. Menopausal status was based on a simple yes-or-no self-report question rather than the more detailed staging system researchers typically use, and only one participant identified as being in the perimenopause transition specifically. Women studied spanned a wide age range, from 20 to 67, which the authors themselves flagged as a limitation for menopause-focused research. Sleep and mood were not directly measured, even though both are common menopause symptoms that exercise could influence and that might have colored the cognitive results. A stretching-and-toning comparison group also included some core-strengthening work, which could have cognitive benefits separate from aerobic exercise. On the hormone side, estradiol levels were not adjusted for menstrual cycle timing in pre-menopausal women, and a related hormone called estrone was not measured. Post-menopausal women using estrogen hormone replacement therapy were excluded at enrollment, but the study had limited information about other medications that can affect estrogen and about whether any participant started hormone therapy during the six-month follow-up.
