Millions of Americans already know that a bad night’s sleep wrecks their morning. But a new study suggests the damage may reach much further, and much further south. Researchers have found genetic evidence that insomnia, longer sleep duration, and daytime napping are each causally linked to a higher risk of developing hemorrhoids, the swollen, painful veins around the lower rectum that affect a large share of the population.

Hemorrhoids are far more common than most people admit. By 2010, they had become the third most common outpatient digestive diagnosis in the United States, generating nearly 4 million doctor visits annually, according to data cited in the study. Yet despite how widespread the condition is, its causes remain poorly understood. Constipation is already considered a major risk factor, and inflammation may also contribute. Now, researchers are making the case that how a person sleeps, or barely sleeps, belongs on that list too.

Published in the journal Neuropsychiatric Research, the study was conducted by researchers at Peking University People’s Hospital in Beijing. Rather than simply asking patients about their sleep habits and health symptoms, which can be unreliable and tangled with other factors, the team used a technique called Mendelian randomization. This approach uses naturally occurring genetic differences between people to investigate cause and effect while reducing some of the confounding and reverse-causation problems that muddy ordinary observational research, though it remains a genetic method with its own assumptions rather than an actual clinical trial.

Genetics as a Stand-In for Sleep Habits

To conduct their analysis, the team pulled genetic data from massive databases containing information on hundreds of thousands of people of European ancestry, primarily from the UK Biobank and a Finnish health database called FinnGen. These repositories contain not just health records, but also data on specific genetic variants tied to sleep behaviors and disorders.

Those genetic variants served as stand-ins for sleep traits like insomnia, total nightly sleep time, and daytime napping. Using statistical methods, the researchers estimated whether genetic variants associated with insomnia also tracked with a higher likelihood of developing hemorrhoidal disease, independent of lifestyle choices or other health conditions. Variants tied to known confounding factors such as body weight, smoking, and alcohol use were screened out to keep the analysis clean. For hemorrhoidal disease alone, the dataset included over 218,000 cases and more than 725,000 controls.

Three Sleep Habits Linked to Hemorrhoid Risk

After running their analysis, three specific sleep patterns showed statistically meaningful links to hemorrhoid risk.

Insomnia showed the strongest connection. People with genetic predispositions toward insomnia had roughly 23% higher odds of developing hemorrhoidal disease compared to those without it. That finding held up even after applying stricter statistical correction, making it unlikely to be a fluke.

Longer sleep duration was also associated with higher hemorrhoid risk, with the genetic analysis estimating about 16% higher odds. That risk tracked with longer sleep, not with sleeping too little. Daytime napping showed about a 14% increased risk. Both associations held up under a standard statistical threshold.

None of the other sleep conditions tested, including sleep apnea and a combined sleep disorders category, showed a statistically meaningful causal link to hemorrhoids or to related conditions like anal fissures and fistulas.

Sleep and Hemorrhoid Risk: A Biological Explanation

On the surface, it might seem odd that what happens in a person’s bed would affect their lower digestive tract. But the connection makes more biological sense than it first appears.

Disrupted sleep throws off the body’s internal clock, the roughly 24-hour cycle that governs everything from hormone release to digestion. When that clock falls out of sync, it can slow the movement of food and waste through the gut, contributing to constipation. Straining during bowel movements is a well-established trigger for hemorrhoids.

Chronic poor sleep also stokes body-wide inflammation. Inflammatory proteins that rise during sleep deprivation can increase the leakiness of blood vessels and promote swelling, the exact problems at the core of hemorrhoidal disease. Daytime napping may act through a separate route the authors propose: people who nap heavily tend to be less physically active, which can impair circulation in the lower body and increase pressure on the veins around the rectum.

One of the more telling findings was what the analysis did not turn up. When the researchers reversed the question and asked whether having hemorrhoids causes people to sleep worse, the analysis found no significant evidence of a causal effect in that direction at the population level. A painful condition disrupting sleep through discomfort alone was not an unreasonable hypothesis, which makes the null result worth attention. While the paper acknowledges that individual patients certainly report sleep disruption from conditions like anal fissures and abscesses, that effect did not register as a detectable signal across the large population studied.

Sleep Advice for People with Chronic Hemorrhoids

For now, the findings suggest that clinicians treating patients with recurring hemorrhoid problems might benefit from asking about sleep habits as part of a broader care conversation. Behavioral approaches to improving sleep, including structured programs that teach people to replace anxious sleep thoughts with healthier habits, have shown effectiveness for insomnia in other health contexts.

Genetic data drove the entire analysis, all of it from people of European ancestry, which limits how broadly the results can be applied. Replicating the findings in other populations and in direct clinical trials would be necessary before sleep management is formally incorporated into standard hemorrhoid treatment guidelines. Still, as a proof of concept, the research suggests that sleep may be another overlooked factor in hemorrhoid risk for the many adults who develop the condition at some point in their lives.
