Smokers who reach for a nicotine vape instead of a nicotine patch are significantly more likely to still be off cigarettes six months later, according to a major updated review from the Cochrane Collaboration.
The analysis, carried out by an international team of researchers and published in the Cochrane Database of Systematic Reviews, pooled data from 80 randomized trials involving nearly 30,000 adult smokers. Cochrane reviews are considered among the gold standards of medical evidence because they gather and analyze all available high-quality research on a given question. This one carries a strong, clear verdict: nicotine e-cigarettes outperform nicotine replacement therapy, the category that includes patches, gum, and lozenges, for helping people quit smoking for at least six months.
For millions of Americans who have struggled to kick cigarettes despite trying every product on the drugstore shelf, that finding matters. Quitting smoking is one of the single most important things a person can do for their long-term health, yet fewer than five percent of people who try to quit on their own stay smoke-free a year later, according to background information in the review.
What the Numbers Actually Mean for Quitting Smoking
When researchers pooled results from 11 studies covering more than 4,100 participants, people who used nicotine e-cigarettes were 61% more likely to have quit smoking at six months compared to those using standard nicotine replacement therapy. In concrete terms, for every 100 people who used nicotine e-cigarettes in an attempt to quit, roughly 8 to 10 succeeded. Among those using nicotine patches or gum, only about 6 out of 100 managed to stay smoke-free. That gap may sound modest in raw numbers, but it becomes meaningful when applied across the tens of millions of people who try to quit smoking every year.
Researchers classified this finding as “high-certainty evidence,” the strongest category of confidence in a medical review. That rating means the researchers are highly confident the true benefit is close to what the data show, not a fluke of small sample sizes or flawed study design.
Nicotine e-cigarettes also came out ahead of nicotine-free versions. People using nicotine-containing devices were about 34% more likely to quit. That comparison matters because it helps answer whether vaping’s quitting power comes from the nicotine or simply from the ritual of holding and puffing a device. It points to the nicotine itself playing a real part, not just the physical habit of vaping. Researchers rated this particular comparison as moderate certainty rather than the highest level.
How Researchers Studied E-Cigarettes and Smoking Cessation
This review is a “living systematic review,” meaning researchers continuously search for new studies and update the findings as fresh data come in. Searches were conducted through January 2026, and this latest update added 9 new studies, bringing the total to 80 completed clinical trials. All 80 were randomized controlled trials, the design considered most reliable in medical research, in which participants are randomly assigned to either use e-cigarettes or a comparison treatment so that results can be fairly compared.
All 29,861 participants were adults who smoked at the time they enrolled. Most studies took place in the United States, which contributed 40 studies, followed by the United Kingdom with 16. Participants varied widely: some were motivated to quit, while others were not particularly interested in quitting at all. Some had serious mental illness or substance-use disorders, while others came from socially disadvantaged backgrounds or were recruited through homeless shelters, making the evidence more applicable to real-world populations rather than just ideal candidates.
Studies measured whether people had stopped smoking for at least six months, a threshold used because short-term quitting is common but long-term success is where the real health gains happen. Many studies confirmed quitting through breath or blood tests rather than relying only on what participants reported.
Are E-Cigarettes Safe Enough to Use for Quitting Smoking?
One of the most pressing questions around e-cigarettes has always been safety. Critics have raised concerns about long-term lung damage, and a widely reported wave of severe lung injuries in 2019 alarmed the public. The review notes that those injuries were later traced specifically to products containing THC, the active compound in marijuana, that had been contaminated with vitamin E acetate. Regulated nicotine-containing e-cigarettes were not the cause.
Within the clinical trials analyzed in this review, serious side effects were rare across all groups. When comparing nicotine e-cigarettes to nicotine replacement therapy directly, the review found moderate-certainty evidence that rates of serious adverse events were similar between the two. For ordinary side effects like throat irritation or nausea, the review found no clear difference between the two groups, though the evidence on those milder effects was less certain.
Compared to people who received only counseling or no support at all, e-cigarette users showed some evidence of slightly more non-serious side effects, though researchers rated that finding as very low certainty, meaning it could easily shift with more data.
Researchers noted that the studies tested regulated, nicotine-containing e-cigarettes. Unregulated, counterfeit, or non-nicotine products may carry very different risks. Not all comparisons in the review carry the same level of confidence, either. For the studies pitting e-cigarettes against counseling or no support, participants obviously knew whether they were vaping or just getting advice, and that awareness can influence how people behave and report their results, which lowered confidence in those findings. A subset of the included studies received funding from tobacco or e-cigarette industry sources; researchers ran additional tests removing those studies and found that the overall results held up without them.
For adult smokers who have struggled to quit using patches, gum, or willpower alone, this review offers high-certainty evidence that nicotine e-cigarettes work better than nicotine replacement therapy. Longer-term safety data are still coming in, and researchers continue to watch for new evidence as the technology evolves. But on the central question the review set out to answer, the verdict is clear: for getting people off combustible cigarettes, a nicotine vape beat the patch.

Limitations
Researchers identified several important limitations in the evidence. For some comparisons, particularly around side effects and safety, results were imprecise because relatively few cases of serious adverse events occurred across all groups, making it difficult to draw firm conclusions. Many of the trials comparing e-cigarettes to counseling or no support could not be blinded, meaning participants always knew which group they were in, introducing a potential source of bias. Most safety findings, including those for non-serious side effects, could change as more studies become available. Additionally, the review did not include unregulated products, illicit e-cigarettes, or devices containing substances other than nicotine, so findings may not apply to those products. Longer and larger trials are still needed to fully evaluate safety over extended periods.
