Exhausted teenage girls often hear the same advice: low iron is probably to blame, so take a supplement. A study of nearly 500 girls in Sweden landed on a more surprising answer. Low iron was common, but it barely predicted which girls felt drained. Heavy periods, skipped meals, and daily nicotine use lined up with their fatigue far more closely, according to researchers at Lund University.

More than half of the girls reported heavy menstrual bleeding, and those girls scored noticeably higher on every fatigue measure researchers tracked, from physical tiredness to brain fog. Girls who used nicotine daily, whether cigarettes or the tobacco-free pouches known as “white snus,” also reported significantly higher fatigue and lower overall well-being.

Iron deficiency and even anemia showed up often in the group, yet neither tracked with how tired the girls actually felt, according to the findings published in the journal Acta Psychologica. The results instead point to the everyday parts of a teenager’s life that rarely make it into a lab report.

What Researchers Found Linked to Teen Girls’ Fatigue

Researchers followed 485 girls ages 15 to 19, recruited from two high schools in southern Sweden. To take part, girls had to have already started menstruating, and researchers excluded anyone with chronic inflammatory illness, ongoing infections, or pregnancy so the results would reflect otherwise healthy teenagers.

Participants filled out a digital questionnaire covering their eating habits, menstrual bleeding, use of nicotine or birth control, and general lifestyle. They also gave blood samples so researchers could measure ferritin, a protein that reflects the body’s iron stores, and hemoglobin, the substance in blood that carries oxygen. Low levels of either can signal iron deficiency or, in more severe cases, anemia.

Fatigue itself was measured using a scale designed for kids and teens that asks about physical tiredness, sleep-related exhaustion, and mental fog. Researchers measured quality of life with a separate questionnaire covering overall well-being and health. Heavy menstrual bleeding was assessed with a screening tool that asks about blood loss and how much it disrupts daily life.

Iron deficiency was common, affecting about 38 percent of the girls, and full-blown anemia affected roughly 8 percent. That’s a lot of teenagers with technically low iron. But when researchers compared fatigue scores between girls with and without iron deficiency, there was no real difference. Same story for anemia. Girls with low iron reported feeling just as tired, or just as fine, as girls with normal iron levels.

Meanwhile, the 54 percent of participants who reported heavy menstrual bleeding again stood out, with higher fatigue scores across the board. Skipping breakfast or school lunch showed a similar pattern, and the most exhausted girls regularly skipped both meals. Daily nicotine users, whether they smoked or used pouches, reported both higher fatigue and lower quality of life than girls who used no nicotine at all.

Perhaps the most unexpected twist: after researchers adjusted their statistical models to account for other factors, higher ferritin and hemoglobin were linked to slightly greater fatigue, the opposite of what conventional wisdom would predict. The study’s authors are careful to say this doesn’t mean iron deficiency somehow makes people feel better. It more likely reflects the messiness of real-world data collection, including the possibility that girls already struggling with fatigue may have started supplements or changed their diets before joining the study, which could mask the expected relationship between low iron and tiredness.

Why Periods Deserve More Attention in Teen Girls’ Fatigue

Heavy menstrual bleeding stayed tied to fatigue even after researchers accounted for iron levels, an association that may reach beyond iron deficiency alone. Researchers point to everyday burdens that often accompany heavy periods, including pain, disrupted sleep, worry about leaks, and the hassle of planning life around bleeding, any of which could wear a person down regardless of iron status. Still, the study didn’t measure which of these might explain the tiredness.

Researchers say this finding lines up with some earlier work, though not all of it, and argue that menstrual health deserves more attention when doctors or parents try to figure out why a teenage girl feels drained all the time. Even when blood tests come back looking normal, heavy periods might still be the more relevant clue.

Dietary patterns themselves showed no consistent link to fatigue. Vegetarian, vegan, and red-meat-free eaters were no more tired than girls who ate meat, even though earlier work from this same research group found that plant-based eaters tended to have lower iron stores. In other words, eating less iron didn’t translate into feeling more exhausted.

This study has clear limits. It couldn’t account for factors like sleep quality, physical activity, mental health, or economic background, all of which plausibly affect fatigue and weren’t measured here. Because the design captured everyone at a single point in time rather than following them over months or years, it can’t prove that heavy periods or meal skipping actually cause fatigue, only that they’re linked to it. And because girls volunteered for a study specifically about iron and fatigue, those already struggling with these issues may have been more likely to sign up, which could skew the overall picture.

Still, the takeaway is hard to ignore. Fatigue in teenage girls looks less like a single number on a lab report and more like the everyday realities of their lives: whether they’re eating regularly, whether nicotine has crept into their routine, and whether their periods are draining more than blood. Low iron can still matter for an individual teenager, but this research argues it rarely tells the whole story.

Researchers note that several potentially important factors, including socioeconomic status, physical activity, sleep quality, and mental health, were not measured, and these could influence both fatigue and iron status. The study did not adjust for inflammation because it did not measure C-reactive protein, and inflammation can raise ferritin independently of iron status. The cross-sectional design captured a single snapshot in time, so it cannot establish cause and effect. Because participants were recruited through a study focused on iron deficiency and fatigue, girls already experiencing these issues may have been more likely to join, which could affect how representative the findings are of adolescent girls more broadly. Heavy menstrual bleeding was self-reported, and some participants may have underreported symptoms if they viewed heavy bleeding as normal.
